Terms:

• There is a one-time installation fee of $90.00 for the 12-month plan.  24-month and 36-month plans are installed free of charge.      
• For the 36-month plan, the DSL modem is supplied free of charge.  For the 12-month and 24-month plans, the usage fee for the MVL DSL modem is $150.00.  The modem must be returned to SAMnet when/if the end user decides to terminate their DSL service.   
• Replacement modems are made available by SAMnet when/if the supplied MVL DSL modem malfunctions and becomes unusable.      
• There is an early termination fee of $150.00 if an account is closed before the plan's time is up.  Also, 24-month and 36-month accounts that are terminated early will have an additional true up fee assessed for the time period in which the account was in use.
